RESPONSIBILITIES:
The Certified Nursing Assistant assists the licensed nursing staff by performing routine nursing duties and activities of daily living. The Assisted Living Certified Nursing Assistant is responsible for a consistent group of residents based on resident centered care. Each C.N.A. is responsible for providing hands –on-care, both physical and emotional, as outlined in each resident’s Individualized Service Plan. The C.N.A. is responsible for effectively communicating with residents and families, and notifying the Assisted Living Manager of any changes in condition or concerns. The C.N.A. is responsible for reading the daily log and for making appropriate entries as needed. The C.N.A. is also responsible in dispensing medication and treatment in a safe organized manner. The C.N.A. is responsible for performing all job requirements in a manner exemplifies Life Care Services standard of customer.
  1. Performs individualized personal care and activities of daily living (ADL) documents ADL’s in flow sheets and medical file progress notes.
  2. Participates in the care planning of designated residents and monthly updates.
  3. Review designated assignments.
  4. Provide Medication Management.
  5. Reports all incidents immediately to Assisted Living Manager/Supervisors and complete incident reports Form.
  6. Observes and reports any physical and /or psychosocial changes of conditions to the Assisted Living Manager Report suspected abuse immediately per state regulations.
  7. Obtains documents and reports the resident’s vital signs to licensed nurse as requested.
  8. Transfers, transports and escorts residents as needed.
  9. Answers emergency pendant alarms promptly.
  10. Answers direct and / or respond to phone calls in a timely, courteous and professional manner.
  11. Helps residents maintain independence; promote dignity and physical safety of each residents.
  12. Knowledge of Federal, State and local resident care regulation.
  13. Provide treatments, carry out procedures; collect specimens as instructed.
  14. Reports and records observations and results of treatments, test, procedures and specimens when indicated.
  15. Transfers, transports and escorts residents as needed.
  16. Answers signal lights promptly.
 
Resident/Guest, family and visitor Relations:
  1. Responds to guest/resident, family, visitor’s inquiries, concerns in a timely and professional fashion.
  2. Assists residents when transitioning to / or from Independent Living and Skilled Nursing.
Medication Administration Duties:
  1. Dispense/Administers/assistance/observation with medications and treatments for each resident using the medication administration record and the six rights for Medication Pass.
  2. Medications are passed according to prescribed times.
  3. Documents and initials as medications are given; appropriate documentation is completed for refusal or missed doses.
  4. Reports to all residents’ concerns made while administering the medication to the Assisted Living Manager.
  5. Assess the residents to determine need for “as needed medication” and appropriately documents and reports to supervisor.
  6. Counts all narcotics with another Lead care Manager/C.N.A. each shift.
  7. Considers the following when administering a medication.

General use of medication

Therapeutic action

Usual dosage

Factors modifying dosage and effects

Precaution and contraindications

Antidote, if known

Route and frequency of administration

Signs of deterioration of the medication

 

8.  Checks with the charge nurse if there are any questions regarding any medication.

9. Safely prepares, administers and charts oral, topical, Ophthalmic and inhaler medications. The Lead Care manager who prepares the medicine dispenses the same.

10. Checks and prepares each dose immediately preceding administration.

Safely prepares, dispense and charts oral, topical, eye drops and inhalers. The Lead Care Manager who prepares the medicine dispense/administers same.

11. Follows administration protocol:

 

Checks medication with transcribed or original order.

Checks all information on label or package for accuracy and clarity.

Clearly identifies the resident with proper medication.

Reports medication errors or reactions to medications immediately to the Lead Care Manager/Supervisor or the Assisted Living Manager, who, in turn, reports to the resident’s attending physician and completes the necessary documentation.

Performs accurate pulse and blood pressure and recognizes their significance in drug therapy

Other Duties:

  1. Maintain safe and sanitary work stations and equipment. Practices safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers.
  2. Understands and practices the proper method of attending to and disposing of the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks and hazardous material.
  3. Ensures oxygen tanks are stores safely, and monitors residents using oxygen for compliance of safety standards.
  4. Encourages independence, attendance at activities, restorative programs and rehabilitation.
  5. Attend in-service training and education, as assigned.
  6. Performs specific work duties and responsibilities as assigned by
QUALIFICATIONS:
  1. Maintains a valid license certification as Certified Nursing Assistant in accordance with federal, state and local regulations.
  2. High School Degree/GED required.
  3. Ability to handle multiple priorities.
  4. Ability to understand and follow instructions in English, communicates effectively, and performs simple arithmetic. Ability to document care given.
  5. Competent in organizational, time management skills.
  6. Basic computer skills.
  7. Demonstrates good judgment, problem solving and decision making skills.
  8. Ability to react and remain calm in difficult situations and be considerate.
